In order to become eligible to participate in Goshen Junior High and 6th Grade Athletics, student-athletes and parents must register through FinalForms. If you do not have an account, click the blue New Account on the FinalForms website. Parents must have an email to register their students. Translation options are available on the registration screen. Parents who are registered in FinalForms and have confirmed their account will receive an email invite to create an EventLink account. These accounts are connected with the same username and password.
A physical form dated after April 1 will be required to participate in athletics for the following school year. Because all the forms are being submitted online, a Clearance Card is no longer needed. The Athletic Department will be available at the court/field to verify your athlete’s eligibility at the first practice.
