Tryout Information and Requirements

 Requirements

The following forms must be completed and submitted to the athletic office before athletes are eligible to practice or tryout.  After completion of the required forms, an Athletic Participation Clearance Card will be issued to you from the Athletic Office.  This Clearance Card notifies the Coach of your eligibility and YOU MUST PRESENT this card to your Coach at the first tryout or practice.

 1.  2014 – 2015 Athletic Physical Form (yellow form)

2.  2014 – 2015 Code of Conduct / Proof of Insurance  / Emergency Medical Release Form (gray form)

3.  2014 – 2015 Concussion Signature Form (purple form)

What to bring

Golf Clubs and weather appropriate clothing for outdoors and indoors.

 Tryouts at Goshen Middle School

March 30 at Goshen Middle School 4:00-5:30pm

March 31 at Black Squirrel Golf Course 4:00-6:00pm

 Meet your Coach in the main lobby after school at 4:00pm.  Parents – please pick up your student athlete at Door W.

Team selection posted and season calendar available in the Athletic Office, Wednesday, April 1.
