Tryout Information and Requirements

Requirements

The following forms must be completed and submitted to the athletic office before athletes are eligible to practice or tryout.  After completion of the required forms, an Athletic Participation Clearance Card will be issued to you from the Athletic Office.  This Clearance Card notifies the Coach of your eligibility and YOU MUST PRESENT this card to your Coach at the first tryout or practice.

1. 2016 – 2017 Athletic Physical Form (yellow form)
2. 2016 – 2017 Code of Conduct / Proof of Insurance  / Emergency Medical Release Form (gray form)
3. 2016 – 2017 Concussion/Cardiac Arrest Signature Form (purple form)

What to bring

Golf Clubs and weather appropriate clothing for outdoors and indoors.

Tryouts at Black Squirrel

Friday, March 24 at Black Squirrel 5:00-7:00pm

Monday, March 27 at Black Squirrel 5:00-6:00pm

Team selection posted and season calendar available in the Athletic Office, Tuesday, March 28.
