Requirements:

The following forms (1, 2 and 3) must be completed and submitted to the athletic office before athletes are eligible to participate or tryout.  After completion of the required forms, an Athletic Participation Clearance Card will be issued to you from the athletic office.  This Clearance Card notifies the coach of your eligibility. YOU MUST PRESENT this card to your coach at the first practice.

1. 2017 – 2018 Athletic Physical Form (yellow form)
2. 2017 – 2018 Code of Conduct / Proof of Insurance / Emergency Medical Release Form (gray form)
3. 2017 – 2018 Concussion / Cardiac Arrest Signature Form (purple form)

For practice on January 8^th, 2018 you will need the following:

 Athletic Participation Clearance Card to give to your Coach. Clearance Cards will be issued during lunch in the café hallway January 8^rd or available in the athletic office December 20th-22th. 

Meet your coach in the locker room at 4:00pm

Practice 4:00-6:00pm

Wear t-shirts, gym shorts and tennis shoes
