       Tryout Information and Requirements

 Requirements

The following forms must be completed and submitted to the athletic office before athletes are eligible to practice or tryout.  After completion of the required forms, an Athletic Participation Clearance Card will be issued to you from the athletic office.  This Clearance card notifies the coach of your eligibility and YOU MUST PRESENT this card to your coach at the first tryout practice.

 

       1. 2016 – 2017 Athletic Physical Form (yellow form)

       2. 2016 – 2017 Code of Conduct / Proof of Insurance  / Emergency Medical Release Form (gray form)

       3.  2016 – 2017 Concussion and Sudden Cardiac Arrest Signature Form (purple form)

Cheer Tryout Practice Date

Tryout practice will be held on:

May 18, Thursday, 2017, 5:30-7:00pm – lower gym

May 19, Friday, 2017, 6:00-7:30pm – lower gym

 

Cheer Tryout Date

Judged Tryouts will be held in the lower gym on:

          Saturday, May 20, 2017

          Warm-ups 9:30am in the Lobby

          Tryouts begin at 10:00am
