2016-2017 Tryout Information and Requirements

7^th Grade Tryouts

Tuesday, December 13, 4:00 – 6:00pm upper gym

Wednesday, December 14, 4:00 – 6:00pm upper gym – First Round Cuts

Thursday, December 15, 4:00 – 6:00pm upper gym – Team Selection

8^th Grade Tryouts

Tuesday, December 13, 4:00 – 6:00pm lower gym

Wednesday, December 14, 4:00 – 6:00pm lower gym – First Round Cuts

Thursday, December 15, 4:00 – 6:00pm lower gym – Team Selection

Requirements  The following forms must be completed and submitted to the athletic office before athletes are eligible to practice or tryout.  After completion of the required forms, an Athletic Participation Clearance Card will be issued to you from the athletic office.  This Clearance Card notifies the coach of your eligibility. YOU MUST PRESENT this card to your coach at the first tryout.

1. 2016 – 2017 Athletic Physical Form (yellow form)
2. 2016 – 2017 Code of Conduct / Proof of Insurance  / Emergency Medical Release Form (gray form)

3.  2016 – 2017 Concussion/Cardiac Arrest Signature Form (purple form)

Selected team members will receive a season calendar on December 15. Our mandatory parent/athlete meeting will be held on Wednesday,January 4^th, 6:00pm in the auditorium.
